Kids Essay Contest – Get Dad to the Doctor

Essay Contest Rules Summary (see below for official rules)

Entries must be original and previously unpublished.

Entries must tell a story about why your father or an important man in your life should go to the doctor. 

Contestants must be residents of Illinois and be between 5 – 17 years of age on July 1, 2010.

Entries must be greater than 50 words but not exceed 200 words.

Proofread your work. Proofread it again. Send only your best work.

Technical ability must be consistent with grade level.  Obvious re-writing by an adult will result in disqualification.

Complete the contact information below.  Entries without complete contact information will be disqualified.

Entries containing violent, discriminatory, defamatory and/or sexually explicit material are not considered appropriate for this contest and will not be considered.

Entries MUST be received between 12:01 a.m on Monday July 5th until 4:59 pm on Friday August 6th, 2010.

Entries WILL NOT be returned.  It is important that you maintain a copy for your files.

Winners will be announced on August 13, 2010.  Prizes for the winning contestants will include 4 tickets to a Chicago White Sox game (Grand prize), 1 iPod® (1st prize), and 10 iTunes® gift cards (runners up) worth $10 each.

Decision of the judges is final.

Essay Contest Official Rules

NO PURCHASE NECESSARY.

PROMOTION PERIOD.  Entries MUST be received between 12:01 a.m., Central Daylight Time (“CDT”) on Monday, July 5, 2010 until 4:59 p.m., CDT, on Friday, August 6, 2010 (“Promotion Period”).  The winner will be announced on or about August 13, 2010.

HOW TO ENTER: During the Promotion Period, pick-up an Official Entry Form at MetroSouth Medical Center, 12935 South Gregory Street, Blue Island, IL, 60406 or email info@metrosouthmedicalcenter.com or on a 3×5 card print/type name of the author, age and grade level, name of parent or guardian, complete address including street number and name, city, state and zip code, telephone number, and parent or guardian’s email.  Then tell us a story about why your father or an important man in your life should go to the doctor.   Then mail the completed Official Entry Form or a 3×5 card with your story to MetroSouth Medical Center, 12935 South Gregory Street, Blue Island, IL 60406 or send the email with your story and contact information.

The story must be longer than 50, but less than 200, words, and must be original and previously unpublished.  Proofread the story.  Have a teacher or parent/guardian proofread it one more time.  Entries MUST be written in English.  Multiple submissions involving different men in your life are allowed.. Entries containing offensive, violent, discriminatory, defamatory and/or sexually explicit material or entries which violate the rights of others will not be considered.  Entries WILL NOT be returned.  It is important that you maintain a copy for your files.  Obvious re-writing by an adult will result in disqualification.

ELIGIBILITY: Contestants must be legal residents of Illinois and be between 5 – 17 years of age on July 1, 2010.  Employees of Sponsor and its affiliates and their immediate families (namely, grandparents, parents, siblings) residing in the same household are not eligible.  All federal, state, and local laws apply.

JUDGING:  Each entry will be ranked in the following criteria:  Originality/Creativity (65%), Writing quality consistent with grade level (25%), and Sincerity (10%).  Technical ability must be consistent with grade level.  Entrants whose entry earns the highest overall score will win.  In the event of a tie, winner will be selected based on the Originality/Creativity criterion.  Judges include one or more business or community leaders who are fathers.  Decisions of the judges is final.  Prizes awarded within 30 days of close of contest.

PRIZE: Grand Prize:  4 tickets to a Chicago White Sox game selected by Sponsor.  First Prize:  Apple® iPod®; Second Prizes:  10 Apple® iTunes® gift cards (APR $10 each).  If winner is a minor, prize will be awarded to winner’s parent or legal guardian.  Merchandise prize is awarded “AS IS” and without warranty of any kind, express or implied, including without limitation, any implied warranty or merchantability or fitness for a particular purpose.  Federal, state and local taxes are sole responsibility of winner.  No prize substitution or cash redemption.

LIMITATION OF LIABILITY.  Sponsor has the right to copy, edit, publish, broadcast and use, in whole or in part, the content of any entry, for commercial purposes without compensation.  Winners and their parent or legal guardian agree that (except where prohibited) Sponsor may use winner’s name, picture, portrait, likeness and voice for advertising and promotional purposes without further compensation.  Sponsor, its promotional and advertising agencies, and their respective officer directors, employees, representatives and agents shall have no liability and shall be held harmless by winners for any damage, loss or liability to person or property, due in whole or part, directly or indirectly, by reason of the acceptance, possession, use or misuse of the prize or participation in this Contest.  Any and all disputes, claims and causes of action arising out of or connected with this Contest, or any prizes awarded, shall be resolved individually, without resort to any form of class action, and exclusively by arbitration.  Any and all claims, judgments and awards shall be limited to actual out-of-pocket costs incurred, including costs associated with entering this Contest, but in no event attorney’s fees.  Sponsor reserves the right, in its sole discretion, to cancel or suspend part of all of this Contest should virus, bugs, non-authorized human intervention or other causes beyond the control of sponsor corrupt or impair the administration, security, fairness or proper play of the Contest.  In such event, Sponsor may award prizes in a random drawing from all eligible entries received up to the date of cancellation or suspension.  Sponsor is not responsible for fraud, whether caused by Sponsor, or by any technical or human error which may occur in the processing of submissions which may limit a participant’s ability to participate in the promotion.  Sponsor may prohibit an entrant from participating in the Contest or winning a prize if, in its sole discretion, it determines that said entrant is attempting to undermine the legitimate operation of the Contest by cheating, deception, or other unfair practices, or intending to annoy, abuse, threaten or harass any other entrants or Sponsor representatives.

CAUTION:  ANY ATTEMPT BY AN ENTRANT TO DELIBERATELY DAMAGE THE WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE CONTEST MAY BE IN VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK REMEDIES AND DAMAGES (INCLUDING ATTORNEY’S FEES) FROM ANY SUCH ENTRANT TO THE FULLEST EXTENT OF THE LAW, INCLUDING CRIMINAL PROSECUTION.
